Dubai: Did you lose your child’s birth certificate when you moved, or was it damaged? While losing such a precious document can be stressful, if your child was born in the UAE, you can apply for a new birth certificate through the official website of the Ministry of Health and Prevention (MOHAP).
Applications for replacement birth certificates can be completed online at mohap.gov.ae and processed within 24 hours, with the option to have it delivered to your home or collected through a MOHAP Public Health Center.

According to the MOHAP Call Center, if you do not have a lost or damaged copy of your birth certificate, you must contact the local health authority in the emirate where your child was born to obtain birth certificate details.

How to issue a replacement birth certificate online
Please follow the steps below to apply for a replacement birth certificate:
1. Visit the link: https://mohap.gov.ae/en/services/issue-of-a-replacement-for-lost-birth-certificate and click “Start Service”.
2. Use your login Emirates Pass Or create an account with MOHAP.
3. After logging in, click “Birth Certificate” in the upper left corner of the website, and then select “Replace Lost Birth Certificate”.
4. Next, you have to enter Kayed, which is the birth notification number, and the date of birth of the mother or father to search if the birth certificate is in the MOHAP system. You can find your Qaid number on the electronic birth notice the hospital gives you.
5. If the system finds the record, MOHAP will display the following message:
• baby’s name
• Father’s name
• mother’s name
• Qaid number
• date of birth
• Status – If the birth certificate is in the MOHAP system, it will state “Certificate Issued”.

6. If the information is correct, tap the three dots in the upper right corner of the screen, then tap Add. Then you need to click the three dots again and click Edit. After clicking the Edit button, you will be taken to the Birth Certificate Lost/Replacement Request page.
7. Next, upload a copy of the mother’s or father’s passport and a copy of the birth certificate.
8. After uploading the attachment, click Submit. You will then be directed to the Request Certificate service. Enter the number of copies of the certificate you want in English or Arabic.
9. Next, you can choose to pick up your birth certificate from the MOHAP Public Health Center or have it delivered to your home address.
10. After selecting your preferred option, click Continue Payment.
11. Go to the payment page and pay electronically by debit/credit card.
After payment, MOHAP will review the application and then issue a replacement birth certificate.
According to the MOHAP website, only parents or authorized persons are allowed to pick up the certificate. However, if you are outside the UAE, a copy of the original document is required and the person holding the power of attorney should receive a certificate. If you do not have a relative to collect the replacement certificate, the relevant consulate staff in your country can collect it on your behalf and present a letter authorizing that person to collect the certificate on your behalf. The letter must be stamped by the respective embassy.
