The surgical team at Mediclinic City Hospital uses the da Vinci Xi robot to perform the first major liver resection in Dubai

Mediclinic City Hospital recently performed the first robotic major liver resection in Dubai using the da Vinci robot. The operation was performed by consultant hepatobiliary surgeon Dr. Hemant Vadeyar, who was assisted by consultant surgeons Dr. Faisel Ikram and Dr. Roger Gerjy.

The patient is a 55-year-old woman who underwent laparoscopic colon cancer resection in 2021. She was recently diagnosed with three tumors in the left lobe of the liver during a routine scan at another hospital. The tumors are thought to have metastasized from her primary colorectal cancer. She was subsequently referred to Mediclinic City Hospital for further treatment.

She was evaluated by the hepatobiliary team at Mediclinic City Hospital and her case was discussed at the GI Cancer Multidisciplinary Team Meeting. This is a conference where all the different specialists dealing with gastrointestinal cancers meet weekly to design a treatment plan for each patient. At the meeting, liver resection was recommended to treat these colorectal liver metastases. It was decided that a formal left lobe hepatectomy was recommended due to multiple lesions in her left liver. This will require removing four of the eight sections of the liver.

Hepatectomy is a complex procedure because the liver is close to major blood vessels and has the potential to cause massive blood loss. Therefore, liver resection is best performed in a dedicated center by a specially trained surgical team. The liver is the only solid organ in the body that can regenerate after surgical removal, and if necessary, 60-70% of the liver may be removed, depending on the patient’s condition. After liver removal, the liver returns to near-normal size within six to eight weeks after surgery. The liver team at Mediclinic City Hospital is trained and experienced in performing this type of liver resection.

“After evaluating the patient and studying the CT and MRI scans, we decided to proceed with robotic liver resection. Traditionally, liver resections are performed through an open approach, involving a large incision in the abdomen. This is followed by a hospital stay7 To 10 days, including 1 to 2 days in the ICU,” explained Dr Vadeyar.

He further explained the use of the da Vinci robot for hepatectomy: “The da Vinci surgical robot enables surgeons to perform this complex procedure using small incisions and a minimally invasive approach, resulting in less blood loss and reduced ICU and hospital stay.”

The patient successfully underwent robotic left lobe hepatectomy on August 29. The surgery took 5 hours and was very smooth with minimal blood loss. The patient did not require blood transfusion and recovered well. Since she only has a small incision, she needs the least amount of pain medication. Her liver function recovered quickly and was discharged after three days of hospitalization. She then continued to perform well and made a full recovery from the surgery.
