Doctors at Tawam Hospital in Al Ain performed a complex two-step head and neck surgery to remove the tumor of a 70-year-old skin cancer patient. They used neuromonitoring to ensure the integrity of the facial nerves.
The patient was from Eritrea with basal cell carcinoma, a skin cancer that usually occurs on areas of the skin that are exposed to the sun, such as the face. People living in countries with above-average annual sunshine levels, such as the UAE and Africa, have an increased risk of skin cancer. In fact, skin cancer is one of the most common male malignancies in the UAE, with a prevalence of 14.5%.
Neuromonitoring is commonly used in complex procedures involving the nervous system to assess the integrity of vital functions. The primary goal is to alert surgeons and anesthesiologists of impending injury so that modifications can be made in a timely manner to prevent permanent damage. It is sometimes used to map areas of the nervous system to guide program management.
The complex and unique procedure involved two operations – the first was a 14-hour operation to remove the tumor and repair the defect with a forearm flap. The second stage of surgery is a parotidectomy (removal of the parotid and main salivary glands) using neuromonitoring and simultaneous bilateral neck dissection and removal of periauricular tumors.
Dr Muwaffaq Al Ani, Oral and Maxillofacial Consultant, Tawam Hospital, Abu Dhabi Health Services Company (Seha) said the patient had basal cell carcinoma that had spread widely across the face.
“The complete removal surgery was particularly unique and complex due to the tumor’s proximity to his eyes, brain and facial nerve. As such, neuromonitoring played a key role in the success of this surgery to ensure we maintained the integrity of the facial nerve and prevent facial paralysis.
