Surgeons at a private hospital in Abu Dhabi removed a cancerous tumor the size of a football, weighing 4.65kg and occupying more than 50 percent of the patient’s abdominal cavity. Doctors diagnosed the patient with liposarcoma, a malignant tumor that would snowball within a year if left untreated.

Yusuf Miya Ismail Ustad, a 60-year-old Indian carpenter by trade, has been suffering from mild abdominal pain for the past six months. Although Ustad is a physically active individual with two grown children, he has no lifestyle-related diabetes or obesity issues.

However, after gaining access to healthcare facilities, he decided to investigate his abdominal pain. On October 23, Ustad attended the NMC Specialist Hospital in Abu Dhabi. After an initial assessment, he was referred to surgeon oncology Dr Balaji Balasubramanian, a surgical oncologist at the same hospital.

Dr. Balaji assessed Ustad for vague abdominal pain. On clinical examination, he was found to have a large mass occupying the entire right side of his abdomen. A CT scan confirmed the presence and location of a large tumor. It extends from the lower surface of the liver to the right side of the pelvis. It wraps around the right kidney and pushes the duodenum (the part of the bowel that connects to the stomach) and the right colon (the longest part of the bowel) to the left of the midline. The possibility of liposarcoma (a subtype of soft tissue carcinoma) was considered. Fortunately, further evaluation did not reveal any distant metastases (spread of the cancer).

A multidisciplinary team, including a surgical gastroenterologist, a urologist, and surgeon oncology, discussed his case, and Ustad was recommended surgery to remove the cancerous tumor.

Family including his wife and two children flew from India to support him and he was admitted to hospital for surgery on November 10. The period until his family’s arrival gave the surgical team a reasonable amount of time for preoperative planning.

Dr Nagendranath Chilukukuru, a surgical gastroenterologist and member of the surgical team at NMC Specialist Hospital, said the tumor was very large and was closely associated with four basic structures: the colon, duodenum, right kidney and a major vessel called the inferior vena cava “

After preserving four vital structures during the marathon 6-hour surgery, the tumor was completely removed with no residue left by the team led by Dr. Balaji, protecting the kidney from surgical damage. The tumor weighed 4.65 kg. The patient lost very little blood and did not need blood transfusion.

Ustad’s case was again discussed by the multidisciplinary tumor board while reviewing various aspects of the literature. It was ultimately decided to follow him closely because the probability of tumor recurrence was approximately 30%. However, his doctors were delighted because no adjacent organs were sacrificed, which improved Ustad’s recovery rate without any surgical consequences.

Despite having such a rare and potentially challenging tumor, Ustad’s surgery went well and did not require ICU care postoperatively. I went home on November 16 and resumed work as planned.

A grateful Ustad was delighted with the success of the surgery, saying: “Cancer is treatable and curable, and my family and I are very grateful to the surgical and nursing teams at the hospital. I really feel better after knowing the size of the tumor removed.” healthy.”
